Position Title: Pharmacist

Division: AA

Location: Eagle Butte, SD, New Mexico

Position Summary

This pharmacist position is to provide pharmacy services in support of the Indian Health Service at Cheyenne River Health Center (CRHC), Eagle Butte, SD. The CRHC provides medical care for approximately 9,000 tribal members living on the Cheyene River Sioux Reservation. The ambulatory clinic provides a full range of medical, dental and community health services on an outpatient basis. The purpose of this position is to provide direct outpatient pharmacy services.

Position contingent upon award of contract, government acceptance of candidate, and positive adjudication of government investigation.

Essential Duties & Responsibilities

CLINICAL DUTIES. The Contract Pharmacist(s)'s primary duties will be processing prescriptions in the EHR, verifying prescriptions for accuracy before dispensing them to the patient, counseling the patients, and documenting counseling and other pertinent patient information in the EHR. The contract pharmacist must be knowledgeable in the following pharmacy functions:

Check for completeness, appropriateness, allergies, and any drug-drug, drug-disease, or drug-food interactions when processing prescriptions, and clarify any prescription order that is inappropriate for dispensing with the ordering prescriber prior to processing.

Assure that all medications prescribed are on the Service Unit formulary or are prescribed in accordance with the local non-formulary policy.

Provide instructions to all prescribing practitioners as to substitutions of medications, as applicable, and answer any medical questions pertaining to medications and patient care.

Keep accurate records of controlled medications.

Participate in Patient Centered Medical Home (PCMH) model and encourage patient engagement.

Establish interpersonal, intra-departmental relationships, and social sensitivity such that he/she can relate constructively to patients from Native American communities to enhance optimal patient care.

Exhibit professional conduct among staff and patients, and respect and maintain the basic rights of patients, to include but not be limited to, personal dignity, cultural sensitivity, privacy, and confidentiality. Violations of these policies may lead to the immediate removal of the Contract Pharmacist from this contract and the Service Unit.

Maintain familiarity and comply with Federal, State, Indian Health Service, accreditation standards, Centers for Medicare & Medicaid Service (CMS) regulations, and departmental regulation and requirements.

Work independently and may also be required to work as the sole pharmacist on certain occasions when the permanent staff is attending meetings or completing administration assignments.

The Contract Pharmacist shall prepare all medical record documentation to meet or exceed the established standards of the IHS Service Unit including, but not be limited to, timeliness, accuracy, content, and signature. All deadlines as determined by the Service Unit shall be met including: 1. All chart documentation shall be completed within 72 hours; 2. Notifications received in the EHR shall be completed within one (1) day; 3. Any deficiencies (documentation that is found incomplete by the HIM department) will be completed within one (1) day after notification.

Education & Experience Requirements

Must be licensed from a state of the United States or its territories to provide pharmacist services.

Must be a graduate of an accredited pharmacy school and possess a BSPharm or PharmD degree.

Must maintain active license to practice pharmacy in any U.S. state through the term of this contract.

Must have 2 years' experience within the last thirty-six (36) months.

Must be able to read, understand, speak and write English fluently.

Must have and maintain current BLS certified throughout duration of contract.

Computer efficiency is required with documented computerized Electronic Health Record experience.

Must submit current immunizations as required by the CRHC immunization policy.

Working Conditions/Working Environment/Physical Demands

Working Conditions/Working Environment: The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. Work Shift: Schedule is Monday-Friday and may include weekends and Federal Holidays. There will be a 30 or 60- minute unpaid meal break included in the schedule.
